Assessment Overview

Introduction to the Assessment

  • Tamar introduces herself and explains that she will be performing an assessment on her mother, covering abdominal, musculoskeletal, and neurological exams.
  • She seeks permission to record the session before starting the examination.

Abdominal Assessment

Inspection and Auscultation

  • The abdomen is inspected; it appears flat, non-distended, symmetrical with no visible peristalsis.
  • Normal active bowel sounds are noted in all four quadrants; no bruits detected in major arteries.

Palpation Techniques

  • Light palpation reveals no pain or tenderness; deep palpation checks for masses and organ size (liver and spleen).
  • Both kidneys are assessed as not enlarged; percussion sounds normal.

Upper Extremity Examination

Inspection and Range of Motion

  • Upper extremities are inspected for symmetry in shoulders, clavicles, and arm length.
  • Range of motion tests for shoulders show no pain during movements such as raising arms or shrugging.

Strength Testing

  • Radial and brachial pulses are checked; both equal at +2. Reflexes tested include biceps, triceps, and brachioradialis.

Lower Extremity Examination

Hip Assessment

  • Hips are examined for symmetry with no reported pain; strength testing shows good bilateral strength when lifting knees against resistance.

Leg and Foot Evaluation

  • Legs inspected for symmetry; palpation reveals no bumps or pain. Knee strength is tested successfully against resistance.

Neurological Examination

Cranial Nerve Testing

  • Cranial nerves I through XII are assessed: smell (cinnamon), visual fields (finger counting), eye movement (convergence), facial sensation/movement, hearing acuity via whisper test.

Coordination Tests

  • Rapid alternating movements tested by finger-to-nose coordination. Heel-to-shin test performed to assess lower limb coordination.

Final Balance and Gait Assessment

Standing Balance Test

  • Patient stands with eyes closed to test balance for five seconds while feet together.

Gait Analysis

  • Walking test includes normal walking, walking on toes/heels, heel-to-toe walking resembling a sobriety test.
