Patient Assessment Overview
Introduction to the Assessment
- Tiffany introduces herself as the nurse practitioner conducting an assessment while ensuring patient privacy by not disclosing personal information.
- The assessment will cover multiple systems: head, ears, eyes, nose, throat (HEENT), respiratory system, and cardiovascular system.
Initial Observations
- The patient is alert and oriented (ANO times 4), with appropriate mood and clothing for the situation.
- No signs of jaundice or concerning skin lesions are observed during the initial examination.
Head and Face Examination
Scalp and Facial Symmetry
- Tiffany checks for tenderness on the scalp while assessing for flakiness or lesions; normal findings are noted.
- Facial symmetry is evaluated through cranial nerve assessments (CN V & VII); all movements are symmetrical.
Eyelid and Eye Functionality
- Eyelids appear normal with no swelling or abnormalities; eyelash growth is equal bilaterally.
- Cranial nerve V sensory function is tested using soft and pointy objects; results indicate normal sensation.
Visual Acuity Tests

Auscultation Findings
