How to Report a Hazard in Sarah Plus
Accessing the Reporting Tool
- To report a hazard in Sarah Plus, users must navigate to the My Monesee portal and select the ONS tile, followed by the "report hazard or incident" tab.
Steps to Report a Hazard
- Click on the waffle icon at the top right corner, then select the "report something" tile to access the hazard or incident reporting page.
- When entering a hazard, choose 'hazard' and provide a brief title. Hovering over the 'I' symbol next to each question offers additional guidance for users unsure of what to write.
- Indicate when the hazard was first noticed and specify its location. For mixed-use spaces, select the most applicable type; for campus locations, use the maze map tool or Google Maps for off-campus hazards.
Providing Details About the Hazard
- Describe the hazard clearly so that follow-up actions can be taken effectively. Select an appropriate category from either physical health (like slips/trips) or psychological health.
- After categorizing, choose relevant mechanisms that could cause harm and rate severity by selecting an appropriate box. If confidential information is involved (e.g., medical conditions), indicate this as necessary.
Finalizing Your Report
- Once all fields are completed, click submit. Users will receive a reference number and an email confirmation of their report for future reference.
