Premature Newborns: Understanding Risks and Complications
Definition and Risks of Prematurity
- A newborn is classified as premature if born before 37 weeks of gestation.
- Low birth weight is defined as weighing less than 2500 grams, regardless of gestational age, significantly increasing neonatal mortality risk.
- Neonatal mortality rates are 40 times higher for low birth weight infants and 200 times higher for those under 1500 grams compared to term infants with normal weight.
Physiological Characteristics of Premature Infants
- Premature infants exhibit anatomical and physiological immaturity, necessitating interventions to promote growth and development while minimizing risks.
- Physical characteristics include thin, permeable skin with a large surface area relative to body weight, leading to increased heat loss and dehydration.
- Immature lungs may lack surfactant, causing difficulty in lung expansion and rapid exhaustion, which can lead to apnea episodes due to nervous system immaturity.
Common Pathologies in Premature Newborns
Hyaline Membrane Disease
- Also known as respiratory distress syndrome, it is linked to pulmonary immaturity and low surfactant levels; most common in infants born before 34 weeks. Treatment begins with corticosteroids administered to the mother at risk of preterm labor.
Patent Ductus Arteriosus (PDA)
- Occurs in infants weighing less than 1500 grams due to altered physiological responses affecting duct closure; incidence decreases with increasing birth weight. Diagnosis involves echocardiography or Doppler studies for clinical correlation before treatment initiation. Treatment options include pharmacological closure or surgical intervention if necessary.
Osteopenia of Prematurity
- A common disorder among newborns under 1500 grams caused by inadequate bone mineralization during the third trimester; nutritional support must be initiated early with calcium and phosphorus supplementation. Mild cases are termed osteopenia while severe cases presenting fractures are classified as rickets.
Complications Associated with Premature Birth
Bronchopulmonary Dysplasia (BPD)
- A chronic lung disease characterized by oxygen dependency; risk increases with lower birth weights and presence of PDA or mechanical ventilation needs; prevention through minimizing risk factors is crucial for improving outcomes during pregnancy and delivery.
Retinopathy of Prematurity (ROP)
- An eye condition resulting from abnormal retinal vasculogenesis that can lead to partial or total vision loss; diagnosis occurs via fundoscopic examination, with treatment ranging from observation to laser photocoagulation depending on severity progression.
Necrotizing Enterocolitis (NEC)
