Estos son los cambios que propone Petro en su reforma a la salud | La Silla Vacía

El Revolcón en el Sistema de Salud en Colombia

The discussion revolves around President Gustavo Petro's proposed overhaul of the healthcare system in Colombia, aiming to ensure smoother access to healthcare services and eliminate barriers faced by individuals seeking medical attention.

President Petro's Healthcare Reform Proposal

  • President Petro aims to enhance disease prevention and streamline healthcare access, reducing the need for legal actions like tutelas for medical care.
  • The current healthcare system in Colombia is primarily managed by EPS (Health Promoting Entities), with both private and government-owned entities catering to the population.
  • Nearly all Colombian citizens are affiliated with an EPS, either through contributory or subsidized schemes, ensuring access to a range of medical services.
  • EPS receive funding from the government based on the number of affiliates, known as the capitation payment, which they use to provide healthcare services.

Functioning of EPS and Challenges Faced

This section delves into how EPS operate within the Colombian healthcare system and highlights some challenges faced in providing efficient and timely medical care.

Operations of EPS

  • EPS contract IPS (Health Service Providers) such as hospitals and clinics to deliver medical services to their affiliates.
  • The capitation payment model poses challenges for EPS as they must balance costs while ensuring adequate care for all members, leading to potential delays in service provision.

Issues with Current System and Proposed Reforms

Here, the focus shifts towards identifying shortcomings in the existing healthcare system and exploring President Petro's proposed reforms aimed at addressing these issues.

Challenges and Proposed Solutions

  • Existing issues include delayed treatments, surgeries, and examinations due to financial constraints within EPS.
  • Despite some satisfaction among members with current services, there are concerns about inefficiencies. President Petro advocates for increased citizen involvement in decision-making processes within the health sector.

Proposed Changes: Citizen Participation & Preventive Care

This segment outlines specific changes proposed by President Petro regarding citizen engagement in healthcare decisions and a shift towards preventive care strategies.

Citizen Involvement & Preventive Care

  • A new National Health Council is suggested for diverse stakeholder representation in health policy discussions but without binding decision-making power.

New Section

In this section, the discussion revolves around proposed reforms in healthcare services, focusing on the distribution of affiliates to health care provider centers (CAPs) and the management of funds.

Proposed Reforms in Healthcare Services

  • The reform proposes that CPS affiliates be distributed among CAPs for medical and dental appointments. Affiliates can visit the nearest CAP for basic care and be referred to hospitals or clinics for specialized treatments like surgeries or examinations.
  • The proposal suggests redirecting funds from CPS to hospitals and clinics instead of EPS. This shift could lead to the disappearance of CPS as they would no longer receive their current funding.
  • There is a plan to divide regions into seven territories to manage funds for healthcare centers under territorial funds managed by local governments. Funds would be allocated through a per capita payment unit annually per patient, unlike CPS which operates based on procedures performed by clinics or hospitals.

New Section

This segment delves into challenges faced by residents in remote areas accessing healthcare services due to distance from medical facilities, along with concerns raised by traditional political parties regarding the sustainability of proposed reforms.

Challenges Faced by Residents in Remote Areas

  • Residents in remote regions encounter difficulties accessing basic care or specialized treatments due to distant healthcare facilities.
  • Traditional political parties such as U, Liberal, and Conservative have expressed doubts about the financial sustainability of the reform proposed by Petro and Corcho.

New Section

The focus shifts towards financial aspects of the reform proposal, including concerns about its sustainability and strategies suggested to manage expenditure efficiently within healthcare systems.

Financial Aspects and Expenditure Management

  • Questions arise regarding whether the reform is financially sustainable given that EPS currently manage funds within set limits provided by the government.
  • Strategies like negotiating lower costs with hospitals for specific procedures are proposed to ensure efficient fund utilization. Petro's model suggests fixed payments to hospitals per procedure based on regional variations.

New Section

This part discusses potential challenges related to controlling expenditure within regional health funds managed by local governments, highlighting risks associated with mismanagement and corruption.

Challenges in Controlling Expenditure

  • Concerns are raised about ensuring effective control over expenditure within regional health funds managed by local authorities.

Video description
