Procedure for Catheterization in Male Patients
Materials Required
- Sabrina Soledad, a third-year nursing student, outlines necessary materials for urinary catheter placement:
- Urinary catheter
- Sterile gloves
- Closed sterile field packs
- Antiseptic solution
- Lubricant
- Syringes and sterile distilled water or saline solution
- Collection bag with a closed system
Initial Steps in Catheterization
- The procedure begins with confirming medical indications and performing hand hygiene before approaching the patient to explain the process, alleviating any fears. The patient is prepared through perineal hygiene. The male patient is positioned supine. Gloves are donned for examination purposes.
Maintaining Asepsis During Catheter Placement
Ensuring Sterility
- After removing examination gloves and washing hands again, sterile gloves are put on. A sterile field is established over the patient's genital area using antiseptic-soaked gauze held with forceps. This step emphasizes maintaining sterility throughout the procedure.
Inserting the Catheter
- The catheter balloon's functionality is checked by injecting air into it using a syringe. A water-soluble lubricant is applied to prevent urethral injury during insertion while positioning the penis at a right angle before slowly introducing the catheter until urine flow is observed.
Connecting and Securing the Catheter
Finalizing Catheter Placement
- Upon observing urine return, connect the collection bag while ensuring sterility; then lower the penis slightly to facilitate urine flow and check that the collection bag’s valve is closed before inflating the balloon with sterile water via syringe. Confirm correct placement through gentle traction on the catheter after securing it properly without touching surfaces other than designated areas.
Procedure for Female Patients
Positioning and Preparation
- For female patients, they are placed in a gynecological position; examination gloves are worn again as antiseptic-soaked gauze separates labia to visualize urinary openings clearly while maintaining sterility throughout this phase of preparation for catheterization.
Insertion Technique for Females
- Similar steps follow as in male catheterization: checking balloon functionality, applying lubricant, gently inserting the catheter into urethra with slight rotations until urine flows back into view; then connecting it to a collection bag while ensuring all aseptic techniques are adhered to throughout this process as well as verifying urine characteristics post-insertion before securing appropriately on thigh level without contact with floor surfaces or other non-sterile areas.
