Introduction to Triage

  • Triage, derived from the French word meaning "to sort," is essential in emergencies with more victims than rescuers. The primary goal is to maximize benefits for the greatest number of people.
  • In emergency situations, responders must stop, look, listen, and think before acting. Identifying oneself and conducting voice triage is crucial for effective communication.

Triage Tags and Classifications

  • Triage tags can range from simple colored strips to complex cards that become part of a patient's medical record. Each tag color indicates the victim's status: green (minor), yellow (delayed), red (immediate), black (deceased).
  • The first step in triage involves checking airway and breathing; over 30 breaths per minute indicate shock, tagging these victims as immediate. If no breaths are detected after two attempts, they are tagged black (deceased).

Assessing Circulation and Mental Status

  • Circulation assessment uses the blanch test or capillary refill time; if color returns slower than 2 seconds, tag as immediate due to potential shock. Severe bleeding requires immediate control through direct pressure on wounds.
  • Mental status checks involve simple commands like "squeeze my hand." Victims who do not respond should be treated for shock and tagged immediate; those passing all tests are tagged delayed.

Interventions for Immediate Victims

  • Immediate victims require three interventions: airway control using head tilt-chin lift maneuver, bleeding control through direct pressure or elevation of wounds, and treatment for shock by laying them down with elevated feet. Monitor body temperature closely even in warm conditions.

Practical Application of Triage

  • A simulated triage event demonstrates practical application: teams announce their presence while assessing casualties based on respiration rates and mental status responses leading to appropriate tagging decisions (e.g., immediate or deceased).
  • Challenges in triage include limited time/resources; involving survivors can help manage the situation better while avoiding common pitfalls such as poor planning or indecisive leadership during emergencies.
